Reports

  • The Program Manager reports to the Chief Executive Officer.
  • The Hagerstown Administrative/ PRP Director and all clinicians reports to the Program Manager.
  • The client service coordinator and the PRP Case Managers reports to the Administrative/PRP Coordinator.
  • Per COMAR the Rehab Specialist actually reports to the PRP Program Manager.

The Program Director Duties

  • Processes PRP referrals through Value-options for initial authorization.
  • Conducts the intake assessments.
  • Allocates new cases to case managers.
  • Also reviews and audits the contact and progress notes -but the Rehab Specialist signs.
  • Organize groups and plans outings for the PRP Clients.
  • Ensures that the Rehab Specialist completes all re authorizations within every six months and updates IRPS.
  • Ensures all consents and signatures are completed at the initial intake.

The Rehab Specialist Duties

  • Prepares IRPs for referrals and completes authorization on value options.
  • Signs off on contact notes and monthly progress notes.
  • Provides “clinical” supervision for the case managers.
  • Completes re authorization and updates all IRPS every six months.
  • Facilitate groups alongside PRP Director and case managers.

Psychiatric Rehabilitation Program Task Duties

  • Completes a discharge summary and ensures it’s duly filled out and signed within 10 working days of discharge.
  • Coordinates and assists case managers a list of local resources and services that will be helpful for their caseload.
  • Request documentation from other care providers including PCP.
  • Administrative oversight and training of case managers.
  • Ensures that case managers meets their contractual contact requirements.
  • Ensuring overall excellent customer experience for clients.
