Position Purpose/Summary

The primary responsibility of the mental health rehabilitation specialist position under general direction, evaluates the social adjustment or vocational needs of persons with mental or emotional disturbances; develops, implements, and monitors rehabilitation plans; acts as a consultant for other mental health professionals and agencies; and performs related duties as required.

Key Accountabilities

  • Ensure the delivery of client centered services to all clients.
  • Deliver excellent client service to CHS’s stakeholders.
  • Ensure continuous improvement of the CHS’s behavioral health treatment processes and operations.

Duties & Responsibilities.

  • Develop diagnostic procedures to determine clients’ needs.
  • Prepare and maintain records and case files, including documentation such as clients’ personal and eligibility information, services provided, narratives of client contacts, and relevant correspondence.
  • Develop rehabilitation plans that fit clients’ aptitudes, education levels, physical abilities, and career goals.
  • Confer with clients to discuss their options and goals so that rehabilitation programs and plans for accessing needed services can be developed.
  • Monitor and record clients’ progress to ensure that goals and objectives are met.
  • Arrange for physical, mental, academic, vocational, and other evaluations to obtain information for assessing clients’ needs and developing rehabilitation plans.
  • Confer with physicians, psychologists, occupational therapists, and other professionals to develop and implement client rehabilitation programs.
  • Collaborate with clients’ families to implement rehabilitation plans such as behavioral, residential, social, and employment goals.
  • Develop and maintain relationships with community referral sources, such as schools and community groups.
  • Foster a staff culture that supports the CHS values and codes of conduct.
  • Provide assistance and information to CHS management as requested.
  • Compliance and attendance of scheduled clinical supervision meetings.
  • Compliance with all administrative and clinical policies and procedures including HIPPA.
  • Participation in all scheduled staff meetings and quarterly brief with the CEO.
  • Other duties as assigned.

  • Comply with the CHS protocols on the appropriate use of telephone, email and internet facilities.
  • Comply with the principles of risk management in relation to individual and corporate responsibilities.
  • Comply with NMC policies on diversity and equality both in the delivery of services and treatment of others.

Job Requirement

Education: Minimum of a master’s degree in social work, psychology and mental health/rehabilitation counseling or a related field from an accredited college or university.

Special Requirement: In addition to a master’s degree, candidates must have a valid license as a Licensed Graduate Social Worker or Professional Counselor [LGPC or LGSW] or a Licensed Clinical Social Worker or Professional Counselor [LCSW-C or LCPC].

Experience: At least one year of professional experience working in mental health rehabilitation counseling and related social services in a public or certified private welfare or health agency.

Addendum

Successful completion of probation includes the following:

(i) Not having more than 2 late document submission.
(ii) Attending all scheduled Administrative and Supervisory meetings.
(iii) Attending all scheduled quarterly meetings with the CEO.
(iv) Participating and completion of all allocated trainings.
(v) Meeting more than 80% (120 clinical contact) of the required 150 monthly contact (or whatever current caseload might be if less than 25 clients) as outlined in contract by the third month of the probationary period.
(vi) Maximizing weekly group participation and client attendance to meet the minimum COMAR billing requirements.
(vii) Ensuring there are Zero denials for billing purposes.

To pass probation, all of the following conditions must be met.

(i) If all or some of these conditions are unmet, there may be an extension of the probationary period.
